Physicals
Students who participate in extracurricular sports must complete a physical exam. Sports physicals are valid for two years.
Parents/guardians should be sure to bring a copy of the OSAA Athletics Physical Examination Form (available in 12 languages) to be filled out by a health care provider at their student's appointment. These forms can be found on your school's athletics webpage for download or at your Athletics and Activities office.
Reminder: When you have completed the sports physical, please return it to your school's Athletics and Activities office.
Wellness Check
Parents/guardians of athletes and non-athletes alike are encouraged to also consider an annual Wellness Check - a complete medical examination and screen for common health issues.
Wellness Check visits are vital to children's healthy growth and development. Physicians suggest adolescents should have regular wellness visits to make sure they are developing at the expected pace and continue to remain healthy.
During these visits, families may have the opportunity to discuss a child's growth, development, behavior and other health issues with the goal of preventing illness and promoting the best possible health for students.
Note: Families that would like to include a sports physical at a student's wellness check, simply request completion of the OSAA Athletics Physical Examination Form.
Athletic Code of Conduct
Any student who participates in school-sponsored athletics or activities must sign a contract to abide by the School District Administrative Policies and Rules, Athletic Code of Conduct, building rules and team participation/training regulations. All students who participate in athletics and extracurricular activities are expected to be positive role models in their communities and beyond at all times. This signed participation contract shall be in effect at all times during the contract period. "At all times" means 24 hours a day, every day of the week whether at school or away from school. The contract will be in effect from the beginning of the OSAA calendar (see www.osaa.org annual dates) and conclude with the last day of high school.
