If you need to request a copy of a student's cumulative file, please contact the student's school where these files are located.

Over the summer months (June - August) please send requests to:

Directory information is defined in the Bend-La Pine Schools Board Policy JOA-AP : Directory Information and JOB-AP: Personally Identifiable Information. To request student directory information, please complete a Student Directory Information Request form. You can fill out and submit the form online, or you can print the form and fax it to Information Technology at 541-355-1210.

We provide transcripts to colleges, universities, trade schools and employers for current and former students. You can also order a transcript for yourself. Please go to our Transcript Requests page for more information.

Request a paper or faxed copy of your transcript for an employer, the Social Security Administration, or other requesting agencies:

• The student requesting the information must complete and sign the form.
• Paper or faxed transcripts can also be requested by contacting the Bend-La Pine school last attended.
• Email questions regarding transcripts to: student-request@bend.k12.or.us

If you are in need of a GED transcript, you can do that directly with GED Testing Service.

If you are in need of a high school diploma replacement, you can do that directly with Miners Graduate Services.

To obtain employment verification for a Bend-La Pine School District employee please contact our Payroll department.
